Making the decision to seek fertility treatments can sometimes be a huge emotional commitment, a commitment that some find difficult to make. At CRH, we have put the processes and staff in place to make sure that when you are ready to make the commitment, we are here to make the experience the best it can possibly be. CRH provides skilled and experienced support staff that is available to help you understand the fertility treatment processes as well as your insurance coverage and potential out of pocket expenses.

We will work closely with your insurance company to best understand how your benefits can be applied to the diagnosis and treatment of infertility. To help you best prepare for your first appointment, you will need to complete your new patient paperwork prior to your arrival. If you did not request a packet by mail, CLICK HERE to print a requisition form to obtain copies of your medical records from your OB-GYN, PCP or previous Infertility Doctor(s) and HERE to access the New Patient Registration forms.

These forms include very important information about you, our organization, and our policies.

Be sure to review and complete all documents and bring them with you on the day of your visit, along with your photo ID and your partner’s photo ID, insurance card(s), and any prior medical records.

You are not alone and there are ways never thought possible to provide you the right therapy for success. Infertility is a common problem and affects 15% of the couples in the US. In our ambulatory surgical center, we are experienced in treating all types of infertility problems.

 

Your First Visit

Here is what your first appointment at CRH will typically look like. You should allocate 2.5 to 3 hours for your appointment.

Check-In - (approx. 25 minutes)

The receptionist will:

  • 1.  Obtain a copy of your driver’s license and insurance card

  • 2.  Collect your completed New Patient Packet

  • 3.  Ask you to complete some additional paperwork (HIPAA Info, etc.)

  • 4.  Build your chart and enter your information into our computer system

Nurse - (approx. 30 minutes)

The nurse will obtain a complete medical history (past, present, family, social history, etc.).

Doctor - (approx. 30-60 minutes)

The physician will do a physical examination and consult regarding any testing or treatment recommendations.

Financial Counselor - (approx. 30 minutes)

The financial counselor/surgery scheduler will schedule any procedures or services and discuss any related out-of-pocket expenses with you.
